Please fill this out if you wish to be a part of our Afterschool Program for the 2023-2024. This is just a pre-registration process to get all necessary information from our participants. Reminder that Annual Memberships are due once per year, if you are unsure of your membership status, you can check when you last paid under "Orders" from your Account Home Screen Menu.
